We seek a PACT Full Time Licensed Master Social Worker (LMSW/LICSW) with a positive, outgoing demeanor for our dynamic team. Responsibilities
  • The social worker will help the patient and family to understand the contributing factors to the problem(s), will discuss with them the pro and cons of possible short term and long-term solutions, encouraging them to make positive and lasting changes to reduce stressors.
  • The social worker will coordinate with clinic staff and community agencies to assist in problem solving as needed. Case management is provided in order to coordinate a collaborative effort to meet the Veteran’s established goals. Services include but are not limited to interventions to increase access to care, including transportation assistance and advanced directives, economic assistance, find appropriate housing for the patient’s level of need (affordable independent housing, homeless shelters, group and family care homes, Veterans Homes, assisted living, nursing homes, respite care, hospice care) and other necessary services to assist with functional decline.
  • Address Advance Directive and Organ Donation in accordance with the standards of CBOC.
  • Provide consultation/education to veterans and families on community resources, advance directives and VA resources.
  • Assists patients and their families with coping, and dealing with the loss and grief experiences in disability, terminal illness and death.
  • Document social work interventions and activities in the patient’s clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care.
  • Fulfill compliance requirements of the Office of Inspector General (OIG), Joint commission (JC), Environment of Care (EOC) oversight, lab compliance and other related items.
  • Clinical Outcome: Remain focused on achieving excellent clinical outcomes through the specified VA guidelines.
  • Must provide excellent customer service to each veteran and his/her family, both in person and over the phone as to fellow colleagues and clinic visitors.
  • Participate in the clinic’s outreach events to help support the clinic’s enrollment initiatives.
  • Support patient enrollment and retention in appointments, follow-up visits or nurse visits.
  • Embrace and support new initiatives, whether clinical or operational.
Qualifications
  • Master's degree, or higher, in Social Work from a program registered by the Department of Education is required, with specific training dealing with adult patients.
  • Must comply with and maintain all requirements for a valid, unrestricted independent MSW (or equivalent) license in the state of California, or in any U.S. state or territory, depending on VA contractual requirements.
  • Recent MSW graduates must apply for the CA LGSW license, obtain 1-3 years post-master direct counseling experience with adult patients Full Time under the supervision of an LICSW
  • Proven mastery of advanced sociological, psychological, and mental health care related interventions and issues.
  • Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
  • Current certification in Basic Life Support from The American Heart Association (must be renewed periodically as specified by the certifying agency – AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contract
  • Proficiency in written and spoken English.
  • Strong computer skills, including EMR experience
