The Patient Aligned Care Team (PACT) is a comprehensive team which delivers primary care services to Veteran patients. The PACT team’s focus is on prevention, health promotion, coordination and chronic disease management. The social worker in this Veteran-centric approach serves on an interdisciplinary team and is assigned to a specific panel of veterans to ensure their needs are met, as defined by the Veteran. Social Workers are uniquely prepared for this role by virtue of their education, training, and experience which prepares them to understand the person-in-situation and the influences that family and community have in impacting the health-care needs of Veterans.


  • Office hours are 8 to 5, weekdays only
  • The social worker will help the patient and family to understand the contributing factors to the problem(s), will discuss with them the pro and cons of possible short term and long-term solutions, encouraging them to make positive and lasting changes to reduce stressors.
  • The social worker will coordinate with clinic staff and community agencies to assist in problem solving as needed. Case management is provided in order to coordinate a collaborative effort to meet the Veteran’s established goals. Services include but are not limited to interventions to increase access to care, including transportation assistance and advanced directives, economic assistance, find appropriate housing for the patient’s level of need (affordable independent housing, homeless shelters, group and family care homes, Veterans Homes, assisted living, nursing homes, respite care, hospice care) and other necessary services to assist with functional decline.
  • Address Advance Directive and Organ Donation in accordance with the standards of CBOC.
  • Provide consultation/education to veterans and families on community resources, advance directives and VA resources.
  • Assists patients and their families with coping, and dealing with the loss and grief experiences in disability, terminal illness and death.
  • Document social work interventions and activities in the patient’s clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care.
  • Fulfill compliance requirements of the Office of Inspector General (OIG), Joint commission (JC), Environment of Care (EOC) oversight, lab compliance and other related items.
  • Clinical Outcome: Remain focused on achieving excellent clinical outcomes through the specified VA guidelines.
  • Must provide excellent customer service to each veteran and his/her family, both in person and over the phone as to fellow colleagues and clinic visitors.
  • Participate in the clinic’s outreach events to help support the clinic’s enrollment initiatives.
  • Support patient enrollment and retention in appointments, follow-up visits or nurse visits.
  • Embrace and support new initiatives, whether clinical or operational.


  • Master’s degree, or higher, in Social Work from a program registered by the Department of Education is required, with specific training dealing with adult patients.
  • Must comply with and maintain all requirements for a valid, unrestricted LICSW license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements.

  • Recent MSW graduates will need to apply for the MN LGSW license, have two years of full time work under the supervision of an LICSW, prior to testing for an LICSW license.

  • Specific requirements could vary based on individual VA contract.
  • Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
  • Basic Life Support certification from the American Heart Association (must be renewed periodically as specified by the certifying agency-AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contract.
  • Proficiency in written and spoken English.
  • Strong computer skills, including EMR experience.
  • Energetic and optimistic demeanor.
  • This is considered a safety sensitive position.
